- Sodium hypochlorite is the only common irrigant that dissolves necrotic and vital organic pulp tissue, which is why it remains the primary endodontic irrigant worldwide; broad antibacterial action including against biofilm; dissolves the organic component of the smear layer, complementing EDTA which removes the inorganic component; lubricates the canal during instrumentation; economical 500 ml clinic bottle rather than a small dispensing bottle. At 5.25% the tissue-dissolving rate and antibacterial action are at their highest, which is an advantage in necrotic and heavily infected cases. The corresponding trade-off is greater cytotoxicity if extruded and a measurable reduction in dentine flexural strength with prolonged contact — many clinicians reserve this concentration for specific indications rather than routine use.
Directions to use
Use only under rubber dam isolation — a hypochlorite accident from an unisolated field or from extrusion beyond the apex causes severe pain, swelling, ecchymosis and, occasionally, permanent nerve damage. Draw into a Luer-lock syringe with a side-vented needle, insert passively to 2–3 mm short of working length so the needle never binds, and express slowly with continuous high-volume suction in the chamber. Refresh frequently throughout instrumentation — irrigant volume and exchange matter more than concentration. Never force the plunger against resistance. Never combine directly with chlorhexidine; flush with saline between them. Protect the patient's eyes and clothing and warn the patient about the taste. Store closed, away from light and heat, as the solution degrades on exposure; check the expiry, since potency falls with age. Confirm exact composition, setting times and contraindications against the SafeEndo IFU before publishing. Given the higher concentration, take particular care with needle depth and pressure, and consider a lower concentration in teeth with an open apex, a large periapical lesion, a suspected perforation or a resorptive defect.
